HOPE Compliance Starts with Timely Documentation

With the October launch of HOPE quickly approaching, you are preparing by reviewing CMS documentation and reading blogs and articles. You may even feel comfortable with the generous amount of time CMS gives you to prepare and submit the HOPE files.

Why Timely Documentation Matters
The first 30 days of a patient’s admission are critical. Don’t place yourself at risk of missing the new deadlines. When HOPE goes live, you will be required to perform and submit:
- HOPE Follow-Up Visit (HUV) #1 between days 6–15
- HOPE Follow-Up Visit (HUV) #2 between days 16–30
- Symptom Follow-Up Visit (SFV) within 2 days of any admission or HUV that documents a symptom impact of Moderate or Severe
You may be thinking, “This is already how I care for my patients.” But what if you miss a Symptom Follow-Up? What if you miss a timely HUV? CMS will not be so kind.

You complete an admission visit on Monday for a patient with a Moderate symptom impact for pain. You address the pain, create an order, and leave the patient’s home.
Since you have other patients to see the same day, you decide to finish the documentation later.
That evening, you fall ill and aren’t able to complete the documentation. Maybe it’s late Tuesday before you can muster the energy to catch up. But an SFV is due by the end of the day Wednesday.
Even if your organization has backup procedures, now you’re in an urgent situation. Resources may need to be juggled just to meet compliance.
